Weight management indication if:
- BMI ≥30, OR
- BMI ≥27 with comorbidity (HTN, dyslipidemia, T2D, OSA, CVD)
Wegovy CV indication if:
- BMI ≥27 AND established CVD (prior MI, stroke, or symptomatic PAD)
- (No diabetes requirement.)
Zepbound OSA indication if:
- BMI ≥30 AND moderate-to-severe OSA (AHI ≥15)
T2D indications if:
- Type 2 diabetes diagnosis (no BMI threshold)
NOT eligible if:
- Personal/family history of MTC or MEN-2
- Pregnant or planning within washout window
- Active severe GI disease
- Active eating disorder requiring specialized care
- Under age 12
- Known serious hypersensitivity
