- BMI ≥30, OR BMI ≥27 with weight-related comorbidity
Wegovy CV (March 2024)
- BMI ≥27 AND established CVD. No diabetes required.
Zepbound (weight)
- BMI ≥30 OR BMI ≥27 with comorbidity
Zepbound OSA (Dec 2024)
- BMI ≥30 AND moderate-severe OSA (AHI ≥15)
Ozempic / Mounjaro (T2D)
- T2D diagnosis. No BMI threshold.
Telehealth provider criteria
- BMI ≥27 typical lower threshold, sometimes ≥25 with comorbidity
- BMI 30+ usually accepted without comorbidity
- BMI 35+ streamlined approval
- T2D, prediabetes, HTN, dyslipidemia, MASLD, OSA, PCOS typically qualify as comorbidity
Calculating BMI
BMI = weight(kg) / [height(m)]² OR weight(lb) × 703 / [height(in)]². Imperfect but the criterion in current labels.
Not eligible
- Personal/family history of MTC or MEN-2
- Pregnancy or planning within washout
- Active severe GI disease
- Active eating disorder needing specialized care
- Children under 12
- BMI below threshold with no qualifying comorbidity
