STEP-1 mean (semaglutide 2.4 mg)
14.9% mean weight loss at 68 weeks. 50%+ of patients achieve ≥15% weight loss; 32% achieve ≥20%.
SURMOUNT-1 mean (tirzepatide 15 mg)
20.9% mean weight loss at 72 weeks. 57% achieve ≥20%; 36% achieve ≥25%.
Individual variation
Response varies. Some patients ('high responders') achieve 25–30%; some ('low responders') achieve 5–10%. Genetics, diet, exercise, and adherence all contribute.
Maintenance decision
At one year: continue at maintenance dose if response is satisfactory. Some patients hold at sub-maximum doses. Discontinuation produces regain (STEP-4).
