The evidence

STEP-1 (sema) and SURMOUNT-1 (tirz) document multi-component cardiometabolic improvement: weight, waist circumference, A1c, fasting glucose, BP, lipids. Effects are larger than weight loss alone would predict.

FDA-approved labeling status

No metabolic-syndrome-specific FDA indication. Use through obesity, T2D, or CV-disease indications as applicable.

Practical considerations

Patients with metabolic syndrome often qualify under multiple indications simultaneously. The CV indication (Wegovy, BMI ≥27 + established CVD) may apply if CVD is documented.
